Similar to the way (well not exactly similar) the cataracts are chopped in phaco, hard cataracts such as those belonging to grade 4 and 5 can be safely chopped in the anterior chamber as shown in the video. Then the two pieces can be aligned vertically along the axis of tunnel and safely expressed out through the 6 mm incision.
